Merit Raise Request Form

This form is for all merit raise requests based on performance. All requests are subject to review. If the request is denied, you will be informed. Please understand that once submitted the raise can take up to two pay periods to be seen on a paycheck. You will receive a copy of the submission in your email. This copy should be used to communicate to the person receiving the raise that their raise has been submitted.
