    This is for a discrepancy in non clocked in hours, for example, this form can be used for hours worked at another restaurant or worked outside of the restaurant. This form should be filled out by the manager on duty, instructor of a class, or General Manager/Supervisor where applicable.

    PTO hours can requested with the new PTO form.  

    If you feel you are missing hours, please use the Missing Hours Report.

  • **If the reason for using the payroll variance form is for an issue relating to punching in at your store, a manager should contact the office and inform them of the issues.
